1. Informasjon om terapeutens dokumentasjonsplikt etter NTFs vedtekter § 4
Jeg er informert om at Victoria Eek som IoPT‑traumeterapeut og medlem av Norsk Traumeterapeut Forening (NTF), er underlagt dokumentasjonsplikt etter foreningens vedtekter § 4(f). Dette innebærer at hun plikter å føre dokumentasjon om sitt arbeid med klienter. Opptegnelser skal minimum inneholde:
• navn og adresse
• angivelse av det som ligger til grunn for 1. gangs henvendelse
• tidspunkt for 1. gangs kontakt og datoer for videre terapeutisk kontakt
Opplysningene skal holdes nedlåst og oppbevares minimum tre (3) år etter siste terapeutiske kontakt.
